To help make sure you are a good fit for hospital visits, and to find out a little more about what you do, please fill out the form below: 

Entertainer Form

For anyone who wants to come into the hospital for the purposes of entertaining, we require that you must possess all of the following: 

  • PVG Scheme Confirmation
  • Public Liability Insurance (PLI) of at least £5m
  • Employers Liability Insurance (ELI) of at least £5m
  • A Complaints Procedure
  • A Data Protection Policy
